• American Sign Language & Live Captioning Request Forms

    Our office will help connect Iowa State offices with vendors for ASL or live captioning services for ISU events. This form may also be used to coordinate ASL interpreting or captioning services for a student registered with our office who requires these services.

  • Exam Accommodations Center Referrals

    Instructors wishing to use the Exam Accommodations Center for their exams will need to complete an exam referral through Accommodate. More information can be found on EAC page "Exam Referrals".

  • Digital Course Materials Request Form

    Students with accommodations that require digital course materials can use this form to request digital copies of textbooks through this form.

  • Presentation Request Form

    If your department/office is interested in having a member of the SAS team present to your group, please fill out the request form linked below. During our presentations, we will provide an overview of our office, the purposes of accommodations, and how they are implemented. The presentation runs 20-30 minutes.
