SAS Formal Appeal Form

By submitting this form, you are requesting a formal review of an accommodation decision made by Student Accessibility Services (SAS).

The purpose of this review is to determine whether the accommodation decision was made through an individualized, interactive process and in accordance with applicable disability law and university policy. This review focuses on whether the decision-making process appropriately considered relevant information and whether the outcome was reasonable based on the documentation provided. It is not a re-evaluation of disability status or a new determination based on additional preference or perspective.

  • A formal appeal may be appropriate under the following circumstances:
  • Relevant documentation or information was not considered at the time of the original decision
  • The interactive process was not followed or was incomplete
  • The decision appears inconsistent with established standards, policy, or previous determinations in similar circumstances

A formal appeal is not appropriate in the following situations:

  • Disagreement with or dissatisfaction about the outcome alone
  • Requests to reconsider accommodations based on preference, convenience, or academic expectations
  • Submission of new documentation that was not part of the original review (this should be addressed through a new or updated request process rather than an appeal)

The appeal process is intended to ensure that decisions are made fairly, consistently, and in alignment with legal and institutional requirements.
